Beta Testing

Important

This is a simple functionality beta test for a simple user registration process. We will trace the critical steps in the process via the email you use for the test, so it is important that the SAME email is used throughout the test.

Instructions for Beta Test Users

1. Go to https://arthureugene.com

1. Find and Click the "Complete Assessment Form" Button
  - Click on Continue

1. Start the Assessment Form
  - Email required field
  - if you need to rectify a mistake on the form, use the up/down arrow or refresh page
  - Takes 4 minutes to complete
  - You MUST use the same email for entire exercise
  - You DO NOT need to use real data for the rest of the form.

1. Check Your Email for Confirmation

1. Use Instructions in your email to register on the AE Portal
  - 1. - Check Spam folder if not in your inbox

1. Register an Account to Join the Portal
  - Use the same email as before - Email required field

1. Complete and Submit the Medical Records
  - Use the same email
  - First Name, Last Name Required*
  - You DO NOT need to fill the rest of the form

1. END TEST

A Survey will be sent to your email for Feedback
